Two practices in Sunderland applied to merge into one practice to ensure their future sustainability to deliver high quality and safe primary health care to their patients. Both practices are based at Southwick Health Centre.

All patients were contacted and invited to share their thoughts and views on the proposed merger, either by speaking with their practice, or submitting thoughts and comments online. A drop-in information session took place on Tuesday 10 December between 3 – 6pm at Southwick Health Centre.
