Both the current providers at Church View Medical Centre and Colliery Medical Group have contacted NHS Sunderland CCG to say that they no longer wish to provide GP services after 30^th September 2017. This means the CCG will need to seek a new provider for the services.

As the two practices are in a similar situation and are located next door to each other, the CCG decided to merge the two contracts and to have one provider deliver GP services for all patients at both practices. Services will not change and you will still be able to access your GP as usual.  We would like to reassure you that you don’t need to take any action and you can continue to access your GP in the normal way.
